To use our payment form, please find your Order Number and Account Number listed on the invoice you received from Vision Plus of Ballard and enter that information in the lower-right portion of this page. On the following page, enter the patient balance listed on your invoice. Then, proceed through the checkout process. We will apply your payment to your account, and you will be sent an automatic receipt through the PayPal Checkout site after your payment has processed.
